Pathophysiology Behind GAE

Abnormal neovessels develop in the synovial lining of the knee joint, delivering inflammatory mediators that perpetuate discomfort.

GAE targets this vascular-inflammatory mechanism directly by selectively embolizing hypervascular genicular branches responsible for synovial inflammation. The goal is not to alter joint structure but to reduce inflammation-driven pain.

Technical Considerations

GAE is performed using selective angiography to identify abnormal hypervascular genicular arteries. Only arteries demonstrating inflammatory blush are embolized, preserving normal perfusion and minimizing risk.

Most patients require treatment of two to four affected genicular branches during a single session. The procedure is typically performed under local anesthesia with conscious sedation and allows same-day discharge.

Outcomes and Patient Selection

Clinical studies demonstrate meaningful reductions in pain scores and improvements in function among appropriately selected patients. As with any intervention, outcomes depend on patient selection, severity of osteoarthritis, and procedural expertise.

Patients with moderate osteoarthritis and inflammatory symptoms tend to benefit most. Severe structural collapse or mechanical instability may still require surgical management.
